Perhaps a text or an email - something that doesn't require him to talk to you or answer your calls but that he is very likely to read.
Whatever method you try, it is important that you keep trying. Dude has a right to know. My question is, did you know while you were dating? I'm going to give you the benefit of the doubt and say that you found out afterwards, but he might have that question so I'd make sure that you are convincing when you tell him when.
By the way, he's likely to call you back after he reads a text or an email that says, "I've got Herpes." If he's a douche, he'll assume you're being an arse and won't respond. To which I'd suggest you send it more than once. And say that you really need to talk to him or that if he doesn't want to talk, he needs to go get tested.
Eithe way, you're doing the right thing by getting in touch with him. I have no idea why he finds you intolerable and that's not important but the fact that you're being upfront gets you some points. Of course that's ONLY if you didn't know.

Use technology. If you can't get in touch wtih somebody in today's day and age, it's on you, not them.
