I think you're thinking of anabolic steroids. They can cause hypogonadism (sp.?) which shrinks the balls. Testosterone cream might make this dude break out and/or develop hair in new places but, if he's getting it on prescription, then he's probably trying to juice abnormally low levels of testosterone, so his balls should be perfectly normal.
Unless he rubs the cream on his balls. I have no idea what happens then. Probably a fire, or a serious pelvic injury.
