I think it's actually in the weird middle ground, somewhere between a tramp stamp and a monocle. Which I guess it technically is.
In this day and age, a bellybutton ring is only going to shock, say, 8% of the American populace. The rest are fine with it, and anyone who finds a piercing sexy or is into the soft curvy belly part of girls will find it sexy.
In short, taste varies. But I don't think there's a big stigma to the bellybutton ring. Just make sure you're not dangling a gold-plated Luis Vuitton logo and you don't have to worry about the trashiness. Plus, guys tend to agree with their female friends that things are trashy that they have no problem with whatsoever when among their own kind.
Now ask me whether wearing clothes that REVEAL the bellybutton ring are sexy or trashy. Because yes.
