It means "Someone I'm porking" or "Someone I'm trying desperately to pork in the near future." More than a friend, not quite a girlfriend.
Have you ever noticed that a guy can't say "lady friend" without a gleam in his eye, a raised eyebrow and the corners of his mouth curling up in a slight grin? Now you know why. He's talking about the dirty deed. Bumping uglies. Coitus.
"I'm taking my lady friend to see Bon Jovi tonight," a guy will say, leaving the second part of the sentence unspoken: "... and after that I'm gonna give her a few rounds from the ol' hamcannon (wink wink)."
That second part is what he's thinking when he gets that conspiratorial look on his face.
"Lady Friend" is just a nice way to say f**k buddy. Sad but true.
My only amendment to this would be if you are over the age of 60 an dating. I have spoken with older women who find the term "girlfriend" childish and would prefer to be called "ladyfriend" by their suitors. Eh, different stokes...
um, my boyfriend calls me his lady friend. we've been dating over 3 years. and are waiting to bump uglies till we're married. so this is not true in all cases. also, i call him my man friend instead of boyfriend. it's just fun/funny to use a different title sometimes. in our case it has nothing to do with fucking.
